 WatchBP Home A: follow-up on Afib

The WatchBP Home A  device is equipped with an atrial fibrillation detection system. When you are diagnosed with hypertensive and/or having atrial fibrillation the WatchBP Home A(fib) is the best tool to optimize your treatment. When you have atrial fibrillation a physician can decide to prescribe you anti-arrhythmic drugs. These drugs should stop the atrial fibrillation. With the WatchBP home A you can easily verify the working of these drugs, while at the same time monitoring your blood pressure.

Atrial fibrillation (Afib)

For the population at large hypertension is the most important risk factor for atrial fibrillation (Afib). The presence of Afib leads to a 2 to 5 fold increased of cardiovascular morbidity and mortality, in particular, it increases the risk of getting stroke [1]. Detection of atrial fibrillation is highly important as starting treatment in an early stadium significantly reduces the risk of getting cardiovascular events in future. In addition, it can influence the decision of which antihypertensive treatment the patient will receive [1].

The Afib detection system

For reasons as described above it is important to detect Afib at your patient. This can be done by either by ECG or palpation. However, the ECG is expensive and labor intensive whereas palpation is not very reliable[2].

By implementing an Afib detection system in some of their blood pressure monitors Microlife comes up with a convenient, easy and reliable solution for detection of Afib.


Accuracy of the Afib detection system


The accuracy of the Afib detection system has been investigated in two clinical studies [3, 4]. Both studies showed that the Afib detection system is very accurate. Stergiou et al. showed that by taking 3 measurements and 2 of them required to detect Afib the sensitivity was 100% and specificity 89% (kappa 0.86 for agreement with ECG). Wiesel et al. found with the same method a sensitivity of 97% and a specificity of 89%.

This means that during blood pressure measurement atrial fibrillation is detected without any extra efforts: easy, convenient and reliable.

WatchBP home plays a valuable role in the long-term observation and management of controlled hypertensive patients.

In-between office visits, patients can take a single duplicate measurement once per week using the “Usual” mode. One week prior to the next office visit, patients can use the “Diagnostic” mode to collect measurements for 7 days as prescribed by the ESH/AHA guidelines. The WatchBP home device with the stored data should then be shown to the physician at the next office visit. The home-measured data in either “Diagnostic” or “Usual” mode can then be directly compared with the office measurements.

Professor Carole Longson, Director of the NICE Centre for Health Technology Evaluation, said: “The independent Medical Technologies Advisory Committee (MTAC) has produced draft guidance supporting the use of Watch BP Home A, based on the evidence it considered. This evidence indicates that the device can offer advantages in measuring blood pressure and detecting atrial fibrillation and that using the device in primary care could increase the detection rate of atrial fibrillation compared with taking the pulse by hand. This would allow preventative treatment to be given to reduce the incidence of atrial fibrillation-related stroke.

“Using WatchBP Home A is associated with estimated overall cost savings per person screened of between £2.98 and £4.26 for people aged 65 or over, depending on their age. We welcome comments on the draft guidance as part of the current consultation.”
